Facilitates nursing care provided to all residents and includes residents in decision making processes; •Actively engages residents in participation in daily activities which are part of resident’s rhythm of daily life (including desired hygiene, grooming, and mobility); •Assesses resident’s needs at the time of admission and continually evaluates residents’ condition during the duration of their stay and promotes the highest functional level of independence possible (keeping in compliance with state and federal regulations, and the community’s safety procedures); •Leads and directs care partners by providing support, encouragement, and direction as appropriate; •Makes frequent rounds and utilizes the nursing process to identify resident conditions that require further evaluation and treatment by a physician; •Reports significant medical changes to the relevant Team Members, Physician, DON, HSA and designated family members as appropriate; •Responds to emergency situations, carries out physician orders, and documents the event, interventions, and outcomes; •Accurately and completely performs direct nursing services, such as, medication administration, treatments, personal care and other nursing procedures. •Reports and documents vital information on all guests’ paperwork; •Actively engages and participates in the community’s QAPI program; •Completes risk management, progress notes, assessments, weekly summaries, and care plans; •Coordinates the residents’ plan of care with the interdisciplinary team (IDT), physicians, medical personnel, and resident’s family/responsible party; •Interacts with other departments such as laboratory, radiology, dietary, facilities, information technology, pharmacy, social services, activities, etc; •Collects lab specimen for testing, obtains and assesses vital signs, weights, I/O’s, etc; •Makes necessary documentation corrections as a result of medical record audits; •Proactively participates in the move-in and move-out procedures for each resident; •Builds genuine relationships with residents through discussion and review of their history; •Contributes to creating a home environment in each community.
